View and manage Forms and Measurements from the Forms section.
Create custom forms and measurement scales to use in data collection sessions. Custom measurements help align performance metrics with protocol standards.

Shared Measurements
Access the Forms section from the Pinnacle menu.
View Forms and Measurements
View current forms, shared forms, current measurements and shared measurements.
- Click the My Forms and Shared Forms tabs to view forms available to the user.
- Click the My Measurements and Shared Measurement tab to view and manage form measurements.
- Click carets to collapse and expand groups.
Forms
Users have the ability to view and manage forms: My Forms and Shared Forms.
My Forms
My Forms page displays all forms created by the user and shows status (Draft, Active, All), and when forms are shared with other employees.
- Click My Forms tab
- Click Status menu to view form by: Active, Inactive and All.
- Select setting to Display Drafts of forms.
Note: All forms saved by the user appear in the All Forms group on the My Forms page. The All Forms group cannot be deleted.
- Click Add Group button to organize your forms in groups.
- Click Add Form button to create a new form.
- Click the Actions menu for each form type to perform specific actions.

Shared Forms
The Shared Forms page has two sections: My Organization's Shared Forms and Templates from Pinnacle.
- My Organization's Shared Forms - displays forms shared with you by members fo your organization. Recipients of shared forms have the ability to use the forms during data collection sessions and to clone the forms.
- Templates from Pinnacle - displays forms created and automatically shared by Pinnacle. Currently, there is one template shared by Pinnacle: Session Checklist.
Note: Forms shared by the user display on the My Forms page with the shared icon .
- Click the Shared Forms tab. Forms shared with the user display on the page.
- View forms in two sections: My Organization's Shared Forms and Templates from Pinnacle .
- Click the Actions menu and Preview link to view a form as it appears during the data collection session. Click Clone link to clone and save the form to the My Forms page.
Measurements
Users have the ability to view and manage measurements: My Measurements and Shared Measurements.
Note: The system has a built-in Y/N Checklist measurement scale. It is accessible from the Shared Measurements page and cannot be deleted.
My Measurements
The My Measurements page displays all forms created by the user.
- Click the My Measurements tab.
- Click Status menu to view form by: Active, Inactive and All.
- Click the Add Measurement button to create custom measurements.
- Click the Actions menu to Edit, Share and Delete measurements.
Shared Measurements
The Shared Measurements page has two sections: My Organization's Shared Measurements and Templates from Pinnacle.
- My Organization's Shared Measurements - displays measurements shared with you by members fo your organization. Recipients of shared measurements have the ability to use the measurements during data collection sessions and to clone the measurements.
- Templates from Pinnacle - displays measurements created and automatically shared by Pinnacle. Currently, there is one measurement shared by Pinnacle: Y/N Checklist.
