Use the form builder to create customized forms to be used during data collection sessions.

Access the Forms section from the Pinnacle menu.

Add Form
- Click My Forms tab.
- Click Add Form button. The new Form page displays.
- Forms have two sections: Details and Customize Form.
- Details - displays form identifiers: Form Name, Status and Form Description. Measurement and Trial information also displays here.
- Customize Form - displays data fields called containers. Container types include:Observation Text, Section & Pinpoints and Static Text.
Form Details
Enter form information details.
- Form Name*
- Status
- Active - default setting for a new form. Active forms display on the All Forms section of My Forms page and are available for session use.
- Inactive - setting to show a form has been retired from use. Form displays on the MyForms page if search filter is set to Inactive. Inactive forms are editable and can be restored to Active status.
- Form Description - use free text field to enter description of the form.
- Measurements*
- Users have the ability to add single and multiple measurements to a form.

- All active measurements in user’s account display on Measurements dropdown menus.
- All Measurements (active and inactive) display on the Measurement page.
- Measurements are created by users and are customizable to the session.
- Y/N measurement checklist is built into Pinnacle and is ready to use.
- Users have the ability to add single and multiple measurements to a form.
- Weighted Calculation setting
- Use toggle to set weighted calculations for form sections.
- Section measurement totals are weighted in the final score calculation.
- Percent field displays next to section name field.
- Multiple Trials setting
- Use toggle to set multiple trials for the data collection session.
- Enter Minimum and Maximum number trials for session*.
- Can be used for all measurements.
Note: The red asterisk icon * indicates information field is required.
Customize Form
Add Containers to customize your forms.
- Containers hold the information for data targets.
- There are three containers types: Observation Text, Section & Pinpoints and Static Text.
Note: At least one container is required to complete the form.
- Click the
icon on the right side of the screen.
- Select Container type: Observation Text, Section & Pinpoints and Static Text.
- The new container is added to the form below the
icon in the Customize Form section.
- Click
icon to delete the container.
- Click
icon to rearrange position of the container.
Container Types
Observation Text - customizable text fields that are editable during the session.
- Capture free text in the Comments section during data collection.
- Unlimited Observation text fields can be added to forms.
Create Observation Text container
- Click Observation Text link from the container menu.
- The Observation Text container displays.
- Enter Header*
- The Observation Text field is used to capture text during the session and is not active when building a form.
- Click Add Observation Text link to add another placeholder row to the container.

to rearrange position of container on the form.
- Click green delete icon

to rearrange position of text field within the container.

Note: User is able to enter text in Comments field during data collection.
Section & Pinpoints - groupings of observable behavior targets that are measured during data collection.
- Section - group of observable behavior targets.
- Pinpoints - behavior targets.
- Unlimited Sections & Pinpoints can be added to forms.
Create Section & Pinpoints container
- Click Section & Pinpoints link from the container menu.
- Section & Pinpoints container displays.
- Enter Section Name*
- Enter Pinpoint*
- Three pinpoint fields display in a new Section & Pinpoints container.
- Only one pinpoint is required.
- Click Add Pinpoint link to add new pinpoint to the container.
- Unlimited pinpoints can be added in a section.
Section & Pinpoint Settings - options to score target as N/A to the session or exclude target from scoring.
Note: To activate settings, move toggle to the right until it turns green.
- Observers can score as N/A setting
- Gives option to score pinpoint target as N/A (Not Applicable).
- Displays as N/A option on session form.
- Exclude from score setting
- Gives option to exclude pinpoint target from the score.
- Displays Not Calculated under the pinpoint name on session form.
- Both settings can be turned on for the same target.

Static Text - customizable text fields that are editable when creating or editing the form.
- They are not editable during session use.
- Unlimited static text fields can be added to forms.
Create Static Text container
- Click Static Text link from the container menu.
- Static Text container displays.
Note: Text must be entered in the Header or at least one static text field in order to save the container and have it display on the form.
Use to add information about the form such as detailed descriptions or instructions. - Enter Header
- Enter custom text.
- Click Add Static Text link to add new field to the container.

Note: User cannot edit static text field during data collection.
Save as Draft and Publish
- Save as Draft button
- Save forms you are still working on as a draft.
- Form displays on the My Forms page with the status of Draft.
- Publish button
- Publish a form when all desired information is added and the form is ready for use data collection sessions.

Preview Mode - preview form as it will display during data collection.
- Move toggle to the right until it turns green to turn on Preview Mode.
- Move toggle to the left to turn off Preview Mode and return edit page.
- Form displays as it will during a session.
